Continuity: the worker the border erased
Why patient-held keys are what let fifteen million migrant workers carry a health record their employer can never see.

Roughly fifteen million Bangladeshis work abroad and send home nearly twenty-four billion dollars a year. The work is dangerous at documented rates, and the wall between the worker and care is not only distance and cost. It is that health status, in the wrong hands, ends a livelihood. A positive tuberculosis result at permit renewal can cancel the permit, which teaches the worker to hide symptoms and arrive home, years later, mid-disease.
So a health record for this worker has a hard requirement most systems never face. It must be useful to him and invisible to his employer and the destination state. A record they can reach is not a convenience for him. It is a file that can be used against him.
For the migrant worker, a record his employer can reach is not a convenience. It is a self-incrimination file.
This is where patient-held keys stop being a principle and become the product. Because the record is encrypted to keys the worker holds, it is unreadable to anyone he has not allowed, including the people who could deport him for what it says. That is the same property the identity layer gives the factory worker hiding a pregnancy, pointed across a border. Continuity provisions his wallet before he leaves, on the pre-departure registration rail he already has to stand on, and anchors him to a Bangladeshi physician who reviews his voice notes in his own dialect, on his own schedule, invisibly.
Continuity is not telemedicine invented for migrants. It is the same spine: a portable, patient-owned, patient-locked record, with the keys doing the one job that matters most for this population. The worker carries a clinical identity into the Gulf and home again, across the border that used to erase him, and the only person who can open it is the person it belongs to. Take the keys away from the patient and there is no version of this that is safe. Leave them with him, and the record that would have been a danger becomes the thing that finally follows him home.
